Overview: The Weight Loss Surgical Center Referral Reward Program is a service of Weight Loss Surgical Center, LLC. This program is provided for all eligible patients of Weight Loss Surgical Center. All users of this web site agree that their access to and use of this web site are subject to the terms and conditions set forth herein and all applicable laws. Any access is undertaken at the user's own risk. These terms and conditions may change without prior notice.
Eligibility: Eligibility for the Weight Loss Surgical Center Reward Points Program is limited to past and current Weight Loss Surgical Center adjustable gastric band patients. Employees of Weight Loss Surgical Center and affiliated companies may not participate in the program and are not eligible to earn points. Medicare and Medicaid participants are also not eligible per federal regulations. Points earned in the program cease to exist when this eligibility is severed. No credit, carryover, transfer or redemption of Referral Reward Points is available after eligibility is severed.
Change: Weight Loss Surgical Center, LLC reserves the right, without prior notice, to discontinue or change the program at any time.
All questions or disputes regarding eligibility for this program, the eligibility of rewards, the availability of rewards or points, the terms and conditions of the program will be resolved by Weight Loss Surgical Center, LLC at its sole discretion.
Participant Enrollment: Each eligible customer will receive a registration key for enrollment in the Weight Loss Surgical Center Referral Rewards Program. Each customer will create their username and password used to gain access to the Weight Loss Surgical Center Points Program website.
Earning Rewards: The earning and accumulation of Reward Points begins when an eligible participant makes a referral, and that referred person becomes a patient of Weight Loss Surgical Center and successfully completes the adjustable gastric band procedure at one of Weight Loss Surgical Center’s facilities. Eligible participants may also earn points by participating in support groups - although points are not guaranteed solely on the basis of attendance or participation. Points will be awarded in accordance with the rules governing confirmation of referrals and discretionary awards authorized by management and awarded by the Weight Loss Surgical Center Referral Reward Program Administrator. There is no backdating of enrollment to retroactively award Reward Points.
Redemption and Deduction of Points:
- Any Reward Points not redeemed before termination or separation from employment states will expire.
- Reward Points may only be used to redeem merchandise from the On-Line Catalog. Points are not redeemable for cash.
- The Reward Points you accrue do not constitute your property. You cannot transfer Reward Points to another participant. Reward Points have no monetary value except to the extent specifically provided for by a reward.
Claiming a Reward:
- Weight Loss Surgical Center reserves the right to modify the merchandise available in the Reward Points On-Line Catalog at any time and without notice.
- When you have obtained sufficient points, you may redeem an item(s) from those described in the On-Line Catalog.
- Specific merchandise is subject to availability and substitutions may be made based on availability or market conditions.
Statements of Weight Loss Surgical Center Reward Points Earned: This web site provides eligible participants with detail of transactions, as well as Weight Loss Surgical Center Referral Reward point balances.
Termination of Program: Weight Loss Surgical Center, LLC reserves the right to terminate the Referral Reward Program at any time without any prior notice to the participants. If the program is suspended or terminated, any unredeemed Reward Points must be redeemed within one month of the date of termination.
Participant Program Tracking: It is the responsibility of each eligible participant to use their awarded points to redeem items from the Weight Loss Surgical Center Reward Points On-Line Catalog. Awarded points can only be redeemed for merchandise presented in the Reward Points On-Line Catalog.
Promotional Events: From time to time, Weight Loss Surgical Center, at its discretion, may sponsor events intended to promote participation in the Referral Rewards Program. These events may be terminated at any time, without cause or warning.
Taxes: Merchandise redeemed under the program is subject to applicable federal, state and/or local taxes. Payment of taxes resulting from these awards is the sole responsibility of the participant and not Weight Loss Surgical Center, LLC or any of its affiliates or agents. Weight Loss Surgical Center, LLC will report earnings under this program in accordance with federal, state and local regulations.
Shipping and Handling: Merchandise redeemed under the program is subject to shipping and handling charges. The shipping and handling charges are included in the point value associated with each item in the On-Line Catalog and are represented in the overall value of the item.
Privacy Notice: This web site is housed and operated by Online-Rewards on behalf of Weight Loss Surgical Center, LLC. The information collected on this web site will be used by Weight Loss Surgical Center, LLC in connection with the Weight Loss Surgical Center Referral Rewards Program.
